Thank you for making Choice Financial your premier choice for financial solutions! We want to make your move as easy as possible. With our Switch Kit, we have everything you need to close your existing accounts, transfer automatic payments, and change direct deposits.

Once you’ve opened your new Choice Financial checking account:

1. Stop using your old account.
   • Make sure all your outstanding checks and automatic withdrawals have cleared. This can take up to two statement cycles.
2. Move your direct deposits to your new Choice Financial checking account.
   • Fill out form SF 1199A U.S. Government Direct Deposit for Social Security, military pay, VA compensation/pension, railroad retirement, etc.
   • Fill out the Direct Deposit Authorization form for any other direct deposits including:
     • Payroll
     • Retirement/Pension Plans
     • Interest Income
     • Dividends
     • Other
3. Move your automatic payments or withdrawals to your new Choice Financial checking account.
   • Fill out the Automatic Payment/Withdrawal Change Request form for any payments you have authorized including:
     • Electric Company
     • Gas Company
     • Water Company
     • Telephone/Cell Phone
     • Cable Service
     • Mortgage Company
     • Insurance
     • Credit Card
     • Car Loan
     • Gym Membership
     • Other
     *Don’t forget about any annual payments you may have set up as a deduction.
4. Finally, close your old account.
   • Fill out the Close Account form.
   • Complete a Close Account form for each checking and savings account you wish to close at your former bank.
